目的:旨在评估经颅直流电刺激(tDCS)联合上肢机器人虚拟情景训练(ULR-VST)对脑卒中患者认知功能的影响。 方法:选取 2022 年 6 月—2024 年 6 月南京医科大学附属脑科医院治疗的 150 例脑卒中患者作为研究对象,通过随机分配法 划分为常规组(常规认知康复)、tDCS 组(tDCS 治疗)和联合组(tDCS 联合 ULR-VST),每组各 50 例,比较三组患者的 认知功能、行为能力和生活能力。结果:干预 2 周后,三组简易智力状态检查量表(MMSE)评分、蒙特利尔认知评估量表 (MoCA)评分比较,联合组 >tDCS 组 > 常规组(P<0.05);三组执行缺陷综合征的行为学评价(BADS)评分、Fugl-Meyer 上肢运动功能评定量表(FMA-UE)评分相比,联合组 >tDCS 组 > 常规组(P<0.05);三组 Barthel 指数比较,联合组 >tDCS 组 > 常规组(P<0.05)。结论:相比常规认知康复和 tDCS 单独治疗,tDCS 联合 ULR-VST 对脑卒中患者的康复效果更佳, 可有效改善患者的认知功能,提升行动能力,提高生活质量。
Objective: To evaluate the effects of transcranial direct current stimulation (tDCS) combined with upper limb robotic virtual scenario training (ULR-VST) on cognitive function in stroke patients. Methods: 150 stroke patients treated at the Affiliated Brain Hospital of Nanjing Medical University from June 2022 to June 2024 were enrolled. They were randomly divided into the conventional group (receiving standard cognitive rehabilitation), tDCS group (receiving tDCS alone) and combined group (receiving tDCS + ULR-VST), with 50 patients in each group. Cognitive function, behavioral performance, and activities of daily living (ADL) were compared across the three groups. Results: After 2 weeks of intervention, the combined group showed significantly higher scores on the mini-mental state examination (MMSE) and montreal cognitive assessment (MoCA) than the tDCS group and conventional group (P<0.05), with a rank order of combined group > tDCS group > conventional group. Similarly, the combined group outperformed the other two groups on the Behavioral assessment of dysexecutive syndrome (BADS) and Fugl-Meyer assessment for upper extremity (FMA-UE) (P<0.05). The Barthel index scores also followed the same trend: combined group > tDCS group > conventional group (P<0.05). Conclusion: Compared to conventional cognitive rehabilitation or tDCS alone, the combination of tDCS and ULR-VST shows better rehabilitation outcomes, which can effectively improve cognitive function, motor performance, and quality of life in stroke patients.
